Before Your Appointment
We will be contacting you 1-2 days before your appointment for a prescreening. This will be done again at your appointment in person.
Bring a mask or they are provided at the front counter at the front entrance. Then sanitize hands and wait to be called for your appointment.
Every patient will be required to complete a COVID Patient Consent Form.
